(NO TREATMENT WILL BE RECEIVED THIS DAY THIS IS A CONSULTATION ONLY) OUT OF STATE PATIENTS- Please call the office to coordinate consult and service in the same appointment. Consultation fee is fully applied to your treatment if used within 90 days of consult date. EXISTING PATIENTS may book this service as well to re-evaluate with Nurse Eva and make a treatment plan.
